![]() ![]() The exams were performed between June 2008 and August 2010, in a major Brazilian university hospital. In this retrospective cohort, we analyzed the medical files and videofluoroscopy exams of 35 patients (study group) diagnosed with neurogenic dysphagia that showed penetration and/or laryngotracheal aspiration and, for this reason, performed the CTM maneuver. 11Ĭonsidering the controversy on its effects and the relevance of this maneuver as one of the first choices for swallowing rehabilitation, 8 12 13 14 we investigated factors that could interfere with the effectiveness of this maneuver in patients with neurogenic dysphagia. In a study involving patients with Parkinson's disease and dementia, CTM was less effective when compared with the intake of thickened liquids. An evidence-based systematic literature review on the impact of dysphagia treatment on populations with neurological disorders found that CTM was effective in 8 to 50% of cases. ![]() 3 4 5 6 9 10Īlthough the CTM is widely used to prevent aspiration in patients with dysphagia, some studies question its effectiveness. 2 Such positive effects are however controversial, since some authors have reported no significant modifications in what concerns pharyngeal contraction and even point out some disadvantages concerning the bolus flow from the oral cavity to the pharynx. 1 5 6 7 8 The literature supports that patients who experience delays in the swallowing trigger usually benefit from this maneuver, as it significantly reduces the risk of food aspiration immediately prior to deglutition. 1 2 3 4 5 6 7ĭuring the execution of this maneuver we observe narrowing of the laryngeal portion of the pharynx, which often compensates for delay of glottic closure during swallowing and favors a shift of the tongue base toward the posterior wall of the pharynx. It is the most frequently employed postural maneuver in the treatment of neurogenic oropharyngeal dysphagia caused by encephalic vascular strokes and degenerative diseases. The so-called chin-tuck maneuver (CTM) does not modify the physiology of deglutition however, it does change pharyngeal dimensions as to direct the bolus toward the pharynx and esophagus. Results The results suggest that the effectiveness of chin-tuck maneuver is related to the overall degree of dysphagia: the more severe the dysphagia, the less effective the maneuver.Ĭonclusion Chin-tuck maneuver should benefit dysphagic patients with delay in the swallowing trigger, reduced laryngeal elevation, and difficulties to swallow liquids, but is not the best compensatory strategy for patients with severe dysphagia. Methods In this retrospective cohort, we analyzed the medical files and videofluoroscopy exams of 35 patients (19 male – 54% and 16 female – 46% age range between 20 and 89 years old mean = 69 years). ![]() Objective The purpose of this study was to investigate the effectiveness of this maneuver in patients with neurogenic dysphagia and factors that could interfere in it. Introduction The chin-tuck maneuver is the most frequently employed postural maneuver in the treatment of neurogenic oropharyngeal dysphagia caused by encephalic vascular strokes and degenerative diseases. ![]() ![]() Our 308 rifle weighed in at a scant 6.2 pounds with a bare muzzle, and no bipod or magazine. This rifle may also help us understand how the rifle weight plays into the recoil equation, and how that affects muzzle brake performance as a percentage of recoil reduction.įor 308 ammo, I selected the very popular Federal Premium Gold Medal 168gr SMK Match-Grade Factory Ammo. So by using this rifle in the tests, it can help give some context for the amount of recoil we’re talking about. While not all of my readers have had the pleasure to shoot a really high-end precision rifle (like the other 3 test rifles), almost everyone has fired a rifle similar to this. ![]() I consider it my baseline for comparison. This particular one is a Savage Axis SR 308 Win. It is a standard, budget-friendly 308 rifle. The rifle I used is not a precision rifle, but it represents something most people are familiar with. 308 Muzzle Brake Resultsįor our mid-sized 30 caliber cartridge, I had to go with the legendary 308 Win. To learn more about those measurements and hear the cases for both sides, please read the recoil post. Average % Reduction – This is just the average of the other two measures (weighted 50/50). ![]() % Reduction in Peak Force – This is the highest instantaneous peak force recorded through the entire recoil event. It isn’t just the peak force, but the entire “area under the curve” when you graph force over time. It represents the total “size” of the force that I described in the recoil post. % Reduction in Overall Momentum – This is the reduction in overall impulse or total momentum of the rifle at the end of the recoil. For each set of results, I will show 3 pieces of information: Here is an example of the data I recorded for one of the muzzle brakes:Īs I publish the results, I plan to show how much each muzzle brake reduces recoil compared to a bare muzzle. So I backed that down to only be 3 shots per muzzle brake. I started out firing 5 shots per muzzle brake, but as I refined the system and setup, my data became so consistent and repeatable that 5 shots seemed like I was just wasting a lot of good match-grade ammo without adding any value. The results are based on the average of multiple shots fired with each muzzle brake. Honestly, I’m not here to try to convince anyone one way or the other, but simply present the data. As we learned in the recoil post, it seems to be some combination of the two factors. Some experts lean more towards that being related to the total impulse or momentum, and others think it is more linked to the peak force. The big question comes down to what measurement most closely correlates to perceived recoil from the shooter’s perspective. Unfortunately, there is no industry standard for measuring recoil, which means manufacturers often publish whatever makes them look better. Here is the full line-up of muzzle brakes, and you can read more about each in this post.Īs we discussed in the recoil post, when someone claims a product “reduces recoil by 50%” … it usually isn’t clear what they’re talking about. For more info on the test equipment and how the data was gathered, please see the recoil post. The recoil data was collected using high-speed, high-resolution force sensors. H1000 will have more speed with the heavies and it is well documented. This post summarizes the results for the recoil tests ran on 20+ muzzle brakes using two 30 caliber test rifles: the 308 Win and the monster 300 Norma Magnum. Lastly I had tremendous accuracy with H4831 in the various 300 mags I've owned. I don't hunt elk or live near where I could hunt them so I cannot give an educated answer on that. In a 300 winchester start with 70 grains of H4831 with a 195tmk loaded so the bullet just touches. Some of the slow powders like the bullets to have longer bearing surface area so pressure builds quicker so a magnum primer produces a better result. Some powders work better when pressure builds slowly so a standard primer works very well. 215's once I get to 70+ grains of powder. I have had great luck with 9 1/2m's in the 270 Winchester and 7 rem mag running 168's. I would use a magnum primer for winter shooting and regular primers for summer or fireforming/break-in. ![]() ![]() It’s hard not to compare the two alone just based on similar gameplay models, which is why Transistor eventually finds itself in the shadow of Bastion, a game that defined a new experience for many. Transistor mirrors many elements of bastion, as well, in its obtuse story, the isometric view, familiar art style, strong music, use of challenge rooms (which unlock music instead of weapons), and even the use of limiters to increase the game’s difficulty. ![]() Usually there’s not much reason to pit a game against another, but with Supergiant’s claim to fame staked in their only other release of Bastion, they’re faced with the difficult second album. I found myself seeing the Turn system mostly when the game called an emergency mode which happens when the player health reaches zero. Again, an interesting idea, but one which is followed by a brief cooldown period in combat, asking players to make an either/or choice to whether realtime action was preferable. Movement included, orders will accommodate sections of the timeline (think Frozen Synapse), and are executed when committed to the player. The other half of combat is focuses on the “Turn” system, a power which freezes gameplay for tactical planning. In this way, the game almost seems to suggest that a lack of commitment to its systems is an ideal way of approaching gameplay. By the time a powerful enemy shows up, useful combinations are sooner a liability than strong asset. This is an interesting system at first, slowly crippling characters before an outright death, but ultimately defies any carefully crafted abilities that may be keepers. Unfortunately, players lose powers in battle, which must be recollected at equipment checkpoints. Since anything can be stacked on top of another power, there’s the potential to create wasteful or ineffective combinations, which gives the sense of experimentation that can reward or penalize. These enemy/attack match-ups are appointed in a way that make sense to the developer, but quickly wash away when returning to the main game, due to the wide variety of enemy types that appear throughout whereas the player may only assess their equipment at specific checkpoints. Players will occasionally encounter an offsite island where challenge rooms will test their skill with a specific set of abilities matched against a specific type of enemy, a la Bastion’s weapon-specific challenges. Transistor finds difficulty in justifying its the abilities system at play, as the aforementioned depth generally obscures the benefit of using one power over any other in a given instance. Mapped to the face buttons, combat centers around these combined abilities in locked off arenas, in which all enemies must be dispatched in order to progress. Transistor’s gameplay is both deeper and less transparent than Bastion’s, featuring abilities that can be stacked upon each other in order of an attack power, upgrade components, or equipped to the character for passive use. Set in a futuristic city, players take the mold of Red, a female protagonist with a chatty sword sidekick, as they venture on to figure out what happened the night before. What it maintains, however, is a striking combination of aesthetics and audio. While set in a familiar isometric perspective, Transistor abandons Bastion’s disembodied narrator and gains a more tactical combat system. Add to this the beautiful visuals and the clever interplay between image and music and you have an intriguing game that is also fun to play.Supergiant Games’ sophomore effort has finally arrived in the form of Transistor, a PC and PS4 exclusive follow-up to 2011’s Bastion. This game manages to perfectly balance together a well thought out combat system with an intriguing story that develops as you play. As you develop, so the enemies develop keeping the combat challenging all the way through the life of the game. You uncover secrets about the character and the world around her by exploring, finding upgrades and fighting a myriad of digital enemies.Īs the game develops, you can equip your character with a wide range of upgrades that can be applied actively and passively to achieve different weapons and combat advantages for your character. ![]() The game starts by throwing you immediately into a mystery, which involves the main character discovering a talking weapon on a dead body - the Transistor. Running through this is a game about the distractions of social media and the allure of celebrity. It mixes real-time fighting with an unusual turn-based system that allows you to pause time, plan out your combat in advance, before unleashing your powers on to unsuspecting enemies.
